Reason For Request
You will need to provide us with information about your need. Requests for computers or equipment are not automatically approved!
If you are an individual , you must have a Non-Profit sponsor to obtain computer equipment. This is a required field for us to process your request. We do check for a valid Non-Profit sponsor.
The name of the Organization, address of the Organization and the Organization's EIN ( ) are required fields.
Military Veterans should contact their local county Veterans Administration Office for sponsorship.
Whether you are a Sponsor or an individual, you must complete the Reason For Request field.
A valid phone number for the Requesting Person will also be required so that we can verify the need.
You must also provide a valid email address.
Please note that we DO NOT ship computer equipment. You must provide us with name of the person who will be picking up the equipment. Payment for Computer Equipment can be in the form of Cash, CERTIFIED Check or Money Order which is required at the time of pickup.

Please limit your computer requests so that others may benefit from our services. The Computer Ministry reserves the right to limit the number of computers ordered, if it appears that computer requests are being abused.
